Introduction
Patient Journey

We at VericiDx would like to thank the American Kidney Fund for their partnership in this project, and their assistance in providing patient and healthcare professional videos. The videos in the patient journey are meant to bring real life experiences to share with anyone who is searching for information to help them with their own personal journey.

There will be white tabs with a patient or transplant professional’s name. Click on these to see transplant practitioner explanations and patient’s personal experiences with their journey.

Diagnosis of Kidney Disease
Diagnosis/Kidney Disease
Diagnosis of Kidney Disease

This section is meant to educate patients and their caregivers on the signs and diagnosis of kidney disease and to understand what resources are available to help manage kidney disease, to hopefully receive care from healthcare providers that can slow or prevent the progression to kidney failure. If that isn’t possible, steps to be taken when the kidneys are failing. The information provided is an educational guideline for expectations when diagnosed with kidney disease. It is advised that patients discuss recommendations in this part of the Patient Journey with all of their healthcare providers and any other doctors taking care of them, for specific advice about their journey.

Dialysis
Diagnosis/Kidney Disease
Dialysis

Dialysis is a way to help the body perform the job of removing waste products and fluid that the kidneys can no longer perform. There are two major types of dialysis. Patients are reminded to follow the instructions of their healthcare provider and all instructions for use or manuals for any medical equipment used or handled as part of a patient’s dialysis treatment.

  1. A patient’s nephrologist can discuss which hemodialysis option makes the most sense for them.

eGFR below 15 ml/min

  1. Long standing kidney disease usually leads to kidney failure (CKD-5).
  2. Patients will have 3 choices at this time, dialysis, palliative care or pursuing transplant. (see buttons below)
  1. - Patients will require dialysis at this point.
  2. - Patients may be transplanted prior to dialysis if they have a living donor.
  3. - Occasionally patients can receive a kidney transplant from a deceased-donor before needing dialysis.

Palliative Care
Diagnosis/Kidney Disease
Palliative Care (Conservative Care)
  1. When a patient isn’t a candidate for transplant and doesn’t want to go through dialysis, they may choose to allow end-of-life to occur.
  2. Palliative care is intended to help patients enter end-of-life in the most comfortable and humane way possible.
