Referral Back to PCP (Primary Care Physician) and General Nephrologist
  1. Clear and consistent communication between doctors, and between the patient and each doctor, is key in the goal of the best and longest quality of life.
  2. Patients should always feel comfortable letting their doctors know if the instructions they were given are not clear. Doctors are happy to explain further if they know there is confusion or are concerns.
  3. As patients are referred back to their local doctors it will be necessary for the patient to take the lead in coordinating care between them and the transplant team.
